Apron Belly Liposuction: Can Lipo Fix a Hanging Belly?

Short answer: not on its own

Apron belly liposuction can’t remove a hanging fold, because the fold is mostly skin and liposuction removes fat. ASPS says good candidates have “firm, elastic skin”,[1] and Cleveland Clinic says liposuction alone “can’t improve saggy, loose skin”.[2] Surgery that removes skin does that. What liposuction does

Liposuction permanently removes fat cells from targeted areas through small cannulas. Cleveland Clinic says it “isn’t a weight-loss solution and it doesn’t treat excess weight or obesity”. It’s meant for stubborn pockets of fat in people whose skin can shrink back over the treated area.[2,1]

ASPS lists the ideal candidate as:[1]

  • An adult “within 30% of their ideal weight who have firm, elastic skin and good muscle tone”
  • Healthy, without conditions that impair healing
  • A nonsmoker and non-vaper
  • Someone with specific, realistic goals for body contouring

Results take time to show. Cleveland Clinic says swelling hides the change at first, with final results visible within three to six months, and that fat can still be gained in treated or untreated areas afterwards.[2]

Why liposuction can’t remove an apron belly

An apron belly is a fold of stretched skin with fat underneath. Liposuction takes out some fat but leaves every square inch of skin. ASPS’s candidacy page notes that skin “can only contract so much” after liposuction, and that extra skin “might require excision with an abdominoplasty”.[1]

ASPS’s practice parameter makes the same point about loose skin in general: excess skin and folds are “virtually impossible to correct by diet, weight loss, or exercise”.[3] Losing fat by suction is no different. A Reddit user in r/PlasticSurgery puts it plainly:[9]

Can liposuction make an apron belly look worse?

It can make loose skin look looser. If fat is removed from under skin that can’t contract, the same skin covers less volume and can hang or ripple more. No clinical study we found measures this for apron bellies specifically, but it follows from the ASPS point that skin can only contract so much.[1]

This Reddit comment says it directly. It’s a user report, not clinical evidence:

Cleveland Clinic lists “contour deformities” among liposuction’s risks, alongside infection and scarring.[2] Ask any surgeon who proposes liposuction for your abdomen what your skin is likely to do afterwards, and whether they’d recommend skin removal instead.

The same caution applies to non-surgical “body sculpting” sold for loose skin. One Reddit user’s verdict on a muscle-stimulation device for postpartum loose skin:

Liposuction combined with a tummy tuck

This is where liposuction does help with an apron belly. A surgeon can remove the fold with a tummy tuck or panniculectomy and use liposuction on nearby areas to refine the contour. Cleveland Clinic says the two can be done at the same time, though combining them increases healing time and surgical risk.[2,4]

StatPearls notes that when liposuction is planned alongside an abdominoplasty, “the intended areas should be clearly marked” before surgery.[4] Cleveland Clinic suggests that if your goal is to get rid of loose skin, a provider might recommend liposuction along with excess skin removal.[2]

  • Which areas will be suctioned, and which will be excised?
  • Is liposuction priced separately from the tummy tuck?
  • How does combining them change recovery and my complication risk?
  • Would you still recommend liposuction if my skin is very loose?

Mons liposuction

Cleveland Clinic says a tummy tuck and liposuction “don’t lift your pubic area”. Liposuction can reduce fat on the mons, but lifting it takes a monsplasty, which surgeons sometimes add to a tummy tuck or liposuction.[5]

Mons work is usually a separate line item. One Reddit user was quoted about $3,000 extra; that’s a single report, not a typical price:

“he said mons lipo could be included at an extra cost. I think it was an additional 3k and had to be performed in addition to the tummy tuck”

A monsplasty is a cosmetic procedure, and Cleveland Clinic says the incision can take at least eight weeks to heal completely.[5] For the difference between the two areas, see apron belly vs FUPA.

Does insurance cover apron belly liposuction?

Very unlikely. Anthem says liposuction “is considered cosmetic and not medically necessary when used for the removal of excess abdominal fat”. Aetna lists suction lipectomy as cosmetic for indications other than lipedema and lymphedema. A panniculectomy is the procedure insurers consider covering for an apron belly.[6,7]

Which option fits your belly

Pinch test first, then a surgeon. If you can lift a fold of skin that hangs over the pubic area, liposuction alone won’t fix it: you’re looking at a panniculectomy or tummy tuck, possibly with liposuction added. If there’s no fold and your skin is firm, liposuction may be an option.[1,3]

  1. No fold, firm skin, near your ideal weight: liposuction may suit you.[1]
  2. A fold causing rashes or hygiene problems: a panniculectomy, which insurance may cover.[8,7,6]
  3. A fold plus a goal of a flatter contour: a tummy tuck, often with liposuction, paid out of pocket.[3,2]
  4. Not ready for surgery: a garment that holds the fold, and good skin care in the crease.
